Act Registered Nurse

The ACT Registered Nurse is responsible for conducting psychosocial assessments; assessing physical health needs; making appropriate referrals to community physicians; providing management and administration of medication in conjunction with the psychiatrist or psychiatric APRN; providing a range of treatment, and support services. The ACT Registered Nurse is a positive team player that promotes a culture of inclusion and participation.

Required Education and Experience

• Associate degree in Nursing
• Must be licensed as a Registered Nurse in the state of Kentucky
• Must have a valid driver's license, reliable operating transportation, and proof of current auto insurance

Preferred Education and Experience

Qualifications desired:

• Bachelor's Degree in Nursing
• One year providing treatment to individuals diagnosed with serious/persistent mental illness

Essential Functions

• Attend daily collaborative team meetings
• Care coordination for scheduling of clients' medical appointments
• Track and administer clients scheduled long-acting injections
• Work with agency pharmacy for clients' medication refills and delivery
• Provide ACT clients with medication education and disease process education
• Conduct weekly medication management groups
• Participate in rotating on-call schedule for after-hours crisis calls
