Primary Responsibilities Conduct in-depth assessments and create individualized treatment plans. Provide psychotherapy to address mental health needs and psychological aspects of medical issues. Support through case management: Help patients follow through on their treatment plans and connect with resources to promote their recovery and well-being. Maintain detailed and timely documentation in our EHR system.
