Nurse Practitioner

POSITION PURPOSE The Vascular Surgery NP/PA provides health care services under the direction of collaborating surgeons in the Department of Surgery and the Advanced Practice Professional team (APP). Under the direction of the surgeon and the Director of APPs, the NP/PA coordinates the health care plan for the assigned patient population by collaborating with consulting physicians and health care professionals. The NP/PA carries out physician orders and maintains the continuum of care through the coordination and integration of all phases of patient care, guided by progress notes, consults, care plans, critical pathways, managed care guidelines, best practice protocols, evidence‑based care, and the utilization review processes. In partnership with the supervising physician, the NP/PA evaluates care based on quality and core measures, access and cost effectiveness. The NP/PA contributes to excellence in patient care by focusing on age‑specific, developmental and spiritual needs and serving as a role model in patient care, consulting, teaching, and leadership. Employment Type Full time Location TRINITY HEALTH ANN ARBOR HOSPITAL Essential Functions and Responsibilities Formulate and implement medical plans of care for acute management of all surgery (inpatient, emergency department, and consult) patients with collaborating physicians. Assist with evaluation and triage of emergency room patients. Provide direct clinical services for surgical patients according to established protocols, using evidence‑based practice and clinical practice standards, under the supervision of collaborating physicians. Patient work‑up, including history and physical examination with ordering of appropriate laboratory tests, x‑ray studies and other diagnostic studies as indicated. Completion of daily hospital work rounds including pre and post operations and charting of progress notes as required or as assigned. Completion of discharge summaries, chart rounds, staffing conference summaries and consults as directed by service needs. Participation in the care of off‑service patients as indicated by supervising physician request. Order appropriate diagnostic and laboratory data under the direction of the supervising physician and communicate results to physicians and other health care providers. Assess changes in patient status and institute routine and emergency interventions to stabilize patients under the direction of the supervising physician and APP team. Document plan of care and patient progress in medical records and narrative summaries. Evaluate and document patient outcomes and revise the plan under the direction of collaborating physicians in association with other specialty consultants. Help facilitate discharge planning along with patients, families, other members of the health care team and referral agencies. Collaborative Role with Other Disciplines Act as the daily central liaison to all health care team members for patient care concerns. Coordinate speech, physical and occupational therapies. Participate in patient care conferences and peer quality review with collaborating physicians, consulting medical teams, and other health care disciplines. Education Focus Explain the purpose, preparation and potential effects of the procedure to patient and families. Provide pre‑operative teaching and discharge planning to patients and families. Design, instruct, and review information and programs pertinent to the inpatient and outpatient surgical population. Participate on hospital, nursing and medical committees as designated. Serve as a hospital resource to staff, patients, and families. Develop and implement a personal continuing education program to maintain current knowledge of surgical care and associated medical concerns. Maintain good rapport and cooperative relationships. Approach conflict in a constructive manner. Help identify problems, offer solutions, and participate in their resolution. Maintain the confidentiality of information acquired pertaining to patients, physicians, employees and visitors to Trinity Health Ann Arbor. Discuss patient and hospital information only among appropriate personnel in appropriately private places. Follow all HIPAA policies and procedures. Required Education, Experience and Certification/Licensure NP Education: Master’s Degree in clinical nursing from an accredited acute‑care nurse practitioner program with emphasis on area of certification/licensure. NP Licensure: Current certification by a nationally accredited professional nursing organization such as ANCC or AANP. Successfully pass the certification exam from an accredited certifying organization approved by the Michigan Board of Nursing relative to the CNP’s designated specialty. Acceptable certifications: Acute Care Adult Gerontology Acute Care and Adult NP. Alternate certifications prior to January 2019 may be accepted or approved if currently practicing at a Trinity Health facility or if there is adequate clinical experience in subspecialty area. PA Education: Requires graduation from an ARC‑PA accredited PA program. PA Licensure: Current License/Certification as Physician Assistant with the State of Michigan. Experience: Surgical experience preferred, but we are willing to train the right candidate. Required Skills and Abilities Interpersonal skills necessary to initiate and maintain collegial relationships with coworkers and therapeutic relationships with patients and families. Analytic skills necessary to perform principal duties and responsibilities of the job description. Ability to concentrate and pay close attention to detail for up to eighty percent of work time when planning and performing professional nursing care. Ability to walk and stand continuously for 50 percent of time, ability to sit for 50 percent of time.
