Job Title: Physical Therapist (PT) – Home Health (Contract) Location: Salem, AR (Coverage within 50-mile radius) Employment Type: Contract Duration: 13 Weeks Schedule: Monday – Friday, 8:00 AM – 4:30 PM Hours: 32–40 hours per week (flexible) Position Overview We are seeking a licensed Physical Therapist (PT) to provide high-quality home health care services in and around the Salem, AR area. This role focuses on patient evaluations, reassessments, and start-of-care visits, ensuring timely and effective treatment in a home setting. Key Responsibilities Conduct Physical Therapy evaluations and reassessments Perform Start of Care (SOC) visits for home health patients Provide treatment visits as needed Deliver high-quality patient care in a home health environment Ensure timely and accurate documentation (within 24 hours) Collaborate with interdisciplinary teams to support patient outcomes Required Qualifications Active Physical Therapist (PT) license in the state of Arkansas CPR Certification Prior experience in home health settings Experience using Homecare Homebase (HCHB) Preferred Skills & Attributes Strong documentation efficiency and accuracy Ability to manage time effectively and meet productivity standards Commitment to delivering thorough, patient-centered care Work Schedule & Coverage Schedule: Monday–Friday, 8:00 AM – 4:30 PM Coverage Area: Within a 50-mile radius of Salem On-Call/Weekend Requirement: None Productivity & Expectations Target productivity: 24–30 points per week Timely documentation submission (within 24 hours) #J-18808-Ljbffr The Judge Group
