ACT Case Manager: Intensive Community Mental Health

Tcn Behavioral Health Services Inc is seeking an experienced Case Manager in Fairborn, OH to provide intensive case management services for individuals with moderate to severe mental illness. Applicants should have a bachelor's degree in a human/social service field and at least 3 years of experience working with SMD populations. Responsibilities include maintaining clinical documentation, serving as a liaison with other agencies, and participating in supervision and training. This position requires valid Ohio driver’s license and the ability to handle potentially challenging client situations. #J-18808-Ljbffr
