Advocate Health in Illinois seeks a Nurse Admissions Coordinator for our hospice division. You will lead patient access, evaluate prognosis and obtain informed consent for hospice care while coordinating with the IDT and physicians. This full-time role requires an RN license, BLS, and at least 2 years of direct patient care, plus a valid drivers license.
