• Travel Required: Regular travel throughout the service area which will include the following counties:
  • Centre, Columbia, Lycoming, Mifflin, Northumberland, Schuylkill, Snyder, Union
• Schedule: Full time, day hours, 35 hours per week with a flexible schedule

CRI is seeking a compassionate, organized, and self-motivated Adult Autism Waiver (AAW) Supports Coordinator to join our team. This position assists individuals receiving services through Pennsylvania's Adult Autism Waiver with identifying, coordinating, and monitoring the services and supports needed to help them achieve their goals and maintain independence.

This is primarily a work-from-home position ; however, the Supports Coordinator must be available to travel throughout CRI's service area in Pennsylvania to meet with participants, families, providers, and other members of the support team. In-person participant visits are an essential function of this position.

Job responsibilities include, but are not limited to:

As an AAW Supports Coordinator, you will manage a caseload of individuals receiving Adult Autism Waiver services and serve as an important connection between participants, families, providers, and the Office of Developmental Programs (ODP). Responsibilities include:

• Conduct annual assessments and assist with required reassessments.
• Develop and maintain Individual Support Plans (ISPs) using a person-centered approach.
• Coordinate services with participants, families, providers, and members of the individual's planning team.
• Monitor services included in each participant's ISP and ensure supports are meeting identified needs.
• Meet with participants in person at least quarterly and maintain required monthly contact with participants, guardians, or representatives.
• Convene planning-team meetings at least annually as needed throughout the year.
• Maintain accurate and timely case documentation in HCSIS in accordance with program requirements.
• Assist with information needed for level-of-care reevaluations.
• Communicate professionally and effectively with participants, families, providers, coworkers, and other community partners.
• Maintain compliance with Adult Autism Waiver and ODP Regulations.
• Complete other duties and provide support to CRI programs as assigned.

Requirements:

• Bachelor's Degree in Education, Psychology, Social Work, or another related social science field.
• At least three years of experience providing case management services to individuals with disabilities or at least three years of experience working with Individuals with Autism Spectrum Disorder (ASD).
• Strong organizational, interpersonal, communication, problem solving skills.
• Ability to work independently while managing multiple participants, deadlines, appointments, and documentation requirements. 
• Comfort utilizing technology and virtual communication platforms, including Microsoft Word, Excel, Zoom/Teams, and HCSIS.
• Ability to complete required training related to needs assessment, person-centered planning, and Adult Autism Waiver requirements.
• Knowledge of needs assessment, applied behavioral analysis, and person -centered planning.
• Ability to meet all CRI and program-required background check requirements.
• Satisfactory driver's record check, valid driver's license, and reliable vehicle
