Join a team that prioritizes growth, open communication, and the tools you need to make a meaningful impact in the lives of others.Location: Las Vegas Northeast and NorthwestJob Summary: We're looking for a skilled and compassionate LPN/LVN to provide in-home nursing care under the supervision of a Registered Nurse (RN). You'll support patients with daily care needs, monitor their condition, and help ensure safe, high-quality home health services.Responsibilities:Provide nursing care according to the RN-developed Plan of Care.Monitor and document changes in patient condition; report to RN/physician as needed.Assist with activities of daily living to promote patient independence.Educate patients and caregivers within LPN/LVN scope of practice.Maintain accurate EMR documentation.Participate in case conferences and coordinate care with the home health team.Follow infection-control, HIPAA, and company policies.Requirements:Current LPN/LVN license in the state of practice.Graduate of an accredited practical/vocational nursing program.Minimum 1 year acute care experience preferred.Current CPR/BLS certification.Valid driver's license, reliable insured vehicle.Strong communication, clinical judgment, and ability to work independently.Physical Requirements: Ability to lift/transfer patients and perform physical caregiving tasks.
