The Case Manager III provides clinical case management services to adults receiving long-term term services and supports. Evaluate client strengths and needs, complete a comprehensive assessment, determine level of functioning, develop and implement service plans that provide a reliable and safe plan of care that enables clients to achieve a maximum level of health and independence and targets maintaining case stabilization, ensuring independence. Complete administrative duties and meet required program deadlines. Provide presentations and/or community education as determined. Coordinate with and perform advocacy with relevant parties and community agencies. Provide oversight to Case Manager II’s, specifically newly hired staff who are unfamiliar with the CARE system. Provide training to Case Management staff on changes to DSHS policy, management bulletins, and any correspondence altering processes received from ALTCEW.

ESSENTIAL DUTIES AND RESPONSIBILITIES includes but are not limited to the following :

• Adhere to program standards and guidelines and follow case management program protocols.

• Provide long-term care services responsive to the needs of elderly and adults with disabilities who are eligible for Title XIX Program services.

• Conduct face-to-face comprehensive assessments at the client's residence to determine level of care, including determining nursing facility level of care needs.

• Develop ongoing care plans which provide the support necessary to allow clients to live independently.

• Facilitate the coordination of in-home caregivers (Individual Providers and Agency Providers) for client care needs.

• Coordinate with ALTCEW and provide advocacy, coordination, consultation and support to clients, providers and stakeholders across multiple disciplines and systems.

• Perform program charting requirements, correspondence and other paperwork in an accurate and timely manner.

• Authorize service payment through Provider One/Individual Provider One.

• Conduct interdisciplinary case reviews and consultations.

• Establish and maintain a good working relationship with FBH staff and community partners. Perform other related duties as required.

• Provide periodic training to all Title XIX staff, and coordinate training efforts with other case Manager III staff members.

• Assist Title XIX Supervisors in providing oversight to Case Manager II’s, including taking new staff on assessments and helping junior personnel with CARE assessment training instruments.

• Comply with all State WAC requirements and FBH policies and procedures.

• Requires travel between various FBH work sites on a regular or daily basis, either via FBH owned Vehicles and/or personal vehicle.

• Other duties as assigned.

QUALIFIED CANDIDATES WILL HAVE:

REQUIRED:

• Master’s degree in behavioral or health sciences and two years of paid on-the-job social service experience; OR

• Bachelor’s degree in behavioral or health sciences and three years of paid on-the-job social service experience; OR

• Master’s degree in any other field and four years of paid on-the-job social service experience; OR

• Bachelor’s degree in any other field and five years of paid on-the-job social service experience, OR

• High School diploma or its equivalent, and Completion of FBH Case Manager Trainee Criteria a n d one year experience as an FBH Case Manager II.

• Must apply for a Counselor Agency Affiliated – Registered credential from the WA State Department of Health within 30 days of hire.

• Must have an unrestricted and valid driver’s license, be able to pass a motor vehicle screening and driving test, have active car insurance, and be willing to use their personal vehicle in the course of work.

KNOWLEDGE, SKILLS & ABILITIES:

• Ability to communicate and engage effectively with individuals of varied backgrounds, cultures, education level and socio-economic status.

• Knowledge of elder care and disabled adult caregiving issues.

• Ability to perform face-to-face assessments and screenings.

• Knowledge of data entry and computer systems to complete tasks.

• Ability to complete administrative tasks and organize and prioritize tasks.

• Experience with leading large groups and public speaking.

• Community settings include but are not limited to residences, group/congregate living settings, hospitals, shelters, the streets or anywhere in the community at large.

• Occasional frequent travel between locations is required.

• The ability to drive a vehicle, traverse distances up to several city blocks and on various terrains, navigate stairs, get in and out of vehicles frequently is required.

• Daily use of a computer is required.

• May be required to lift 10 - 20 pounds.

• Blood-borne Pathogen Category III - No predictable exposure to blood-borne pathogens.

PAY RANGE (Hourly/ Non-Exempt): $29.26-$35.65

All pay is based on longevity at Frontier Behavioral Health; thus, the starting pay for this position is $29.26 per hour.
