Travel Physical Therapist — Home Health — Hagerstown, MD

Provide skilled physical therapy services to home health patients in Hagerstown, MD. Deliver evaluations, develop and implement individualized treatment plans, and coordinate care with interdisciplinary teams. Document patient progress and discharge plans according to regulatory and agency standards. Maintain professional licensure and required certifications while representing the agency in the community.

Minimum Qualifications

• Bachelor‘s, Master‘s, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
• Active Maryland Physical Therapy license required to start the assignment.
• BLS certification from AHA or ARC may be required.
• Experience providing care in home health settings preferred.
• Ability to perform patient evaluations, create plan of care, and provide skilled therapy interventions independently.
• Valid driver’s license and reliable transportation for travel to patient homes.

Preferred Qualifications

• Previous travel therapy experience is preferred.
• Experience with documentation software and electronic medical records preferred.

Assignment Details

• Potential backfill needed.
• Candidate may be expected to travel to multiple patient residences within the service area.
