Outpatient Physical Therapist

Job Description

The Outpatient Physical Therapist evaluates all assigned patients and develops, implements, and modifies treatment plans according to the patients' needs, goals, and physician directions. The therapist is responsible for the delegation and direction of all discipline-specific therapy services for those assigned patients. They ensure the design of functional treatment programs allowing for effective return to community activities. This role also includes serving as an educator for patients, families, students, coworkers, and self while demonstrating behaviors consistent with the organization's Mission Statement and Core Values.

Responsibilities

• Completes comprehensive evaluations of assigned patients.
• Utilizes clinical reasoning to develop appropriate treatment plans and goals.
• Maintains clear, concise, accurate, and timely documentation in medical records.
• Ensures care is provided under valid physician orders and within insurance approval.
• Participates in the student program and provides a positive learning environment.
• Creates an individualized professional development plan and pursues growth opportunities.
• Adheres to the discipline-specific State Practice Act and maintains licensure in good standing.
• Demonstrates awareness of organizational performance improvement initiatives and participates in them.

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an accredited school.
• 6 months to 1 year of outpatient clinical experience.
• Current CPR certification.
• State licensure for physical therapy in good standing.

Schedule

This position follows a 5x8.0 schedule, starting October 26th, with 60-minute evaluations and follow-ups every 30 minutes.
