Physical Therapist Assistant

Synchrony Rehab is seeking a compassionate and skilled Physical Therapist Assistant to join our Healthcare & Medical Services team dedicated to delivering innovative, patient-centered rehabilitation. In this role, you will work under the direction and supervision of a licensed Physical Therapist to provide high-quality physical therapy interventions that promote optimal recovery, functional independence, and overall well-being for patients across a variety of healthcare settings. You will implement individualized treatment plans, lead therapeutic exercises and activities, apply appropriate modalities, and provide patient and family education to support carryover of care. As a Physical Therapist Assistant with Synchrony Rehab, you will collaborate closely with an interdisciplinary team to ensure coordinated, evidence-based care that aligns with each patient’s goals and abilities. You will assist with initial observations and ongoing assessment of patients’ responses to treatment, document progress accurately and timely in the electronic medical record, and communicate changes in condition or functional status to the supervising Physical Therapist and other care team members. Your role will also include helping to maintain a safe and organized therapy environment, adhering to all regulatory and professional standards, and contributing to quality improvement and outcome-driven initiatives.
