Patient Care CoordinatorWe are seeking a dedicated Patient Care Coordinator in the homecare industry. This role involves managing daily patient care and coordinating caregivers to ensure high-quality service and accurate recording of caregiver clock-ins and outs. Proficiency in programs like HHAx and experience with Smartsheet are required. The ideal candidate is motivated, patient, and proactive.Key Responsibilities:Oversee daily patient care activities and caregiver scheduling.Communicate effectively with patients, families, and caregivers.Adjust care plans as necessary.Ensure compliance with healthcare regulations.Use HHAx for scheduling and tracking.Qualifications:Experience in patient coordination for home care.Skilled in HHAx, Smartsheet, and computer use.Strong communication and problem-solving skills.Proficiency in English and Spanish.If you are committed to improving patient experiences and meet these skills, we encourage you to apply to join our team.
