Posting Summary Rutgers, The State University of New Jersey, is seeking a Patient Navigator in the Department of Infectious Disease within the New Jersey Medical School (NJMS). The primary purpose of the Patient Navigator position is to provide clinic and community‑based HIV, STI testing, and case management services to persons with and at risk for HIV infection. The role includes performing a biopsychosocial assessment, developing an informed care plan, maintaining confidential client information, reporting issues to the Program Manager, and working in clinical settings and on the mobile health unit, including select weekend and evening hours. Key Duties Provides a confidential assessment for clients living with HIV and those referred for PrEP services, determining HIV prevention needs and registering and planning prevention services. Provides HIV and STI confidential pre‑ and post‑test counseling for identified patients and others according to NJ Department of Health and Senior Services and CDC guidelines, documenting accordingly. Provides counseling and testing services at the clinic, outside events, and the mobile health unit. Conducts quality controls daily and weekly (e.g., room temperature, freezer temperature). Familiarizes and maintains knowledge of policies and procedures related to all aspects of STI and HIV counseling and testing. Minimum Education and Experience Bachelor's Degree in Social Work or Human Services plus two (2) years of full‑time professional experience with HIV/AIDS. Equivalent education, experience, and/or training may be substituted for the degree requirement. Certifications and Licenses NJ certification as an HIV Counselor. Valid Driver's license required. Required Knowledge, Skills, and Abilities Must be able/willing to learn to drive mobile unit. Must have completed or be eligible to successfully complete the Rutgers Defensive Driving Training Course. Proficiency in Microsoft Windows‑based Excel, Access, and Word required. Ability to exercise considerable independent judgment utilizing professional level of knowledge to establish work priorities within the scope of responsibility. Excellent and demonstrated customer service skills required. Ability to work varied schedules including weekends/ evenings. Incumbent must fulfill all required job qualifications, including requirements resulting from current/future modifications to federal/state funding guidelines. Preferred Qualifications Five (5) years of experience preferred. Case management experience and Phlebotomy certification preferred. Bilingual skills (English/Spanish) preferred. Position requires lifting up to 10 lbs. No special vision requirements. Work Environment WORK ENVIRONMENT: Moderate noise. Work Arrangement This position requires a fully on‑site work arrangement and may be eligible for a compressed workweek or a flex workday schedule.
