I) Purpose

In Tarrant County, individuals receiving services under HCS, TxHmL, and GR CFC programs are each assigned a Service Coordinator (SC). The SC assists individuals in identifying and expressing their goals, needs, and preferences, ensuring that services align with their Person Directed Plan (PDP). Disability Services aims to deliver consumer-driven services seamlessly, and the SC plays a crucial role in coordinating and monitoring these services. Additionally, the SC must possess knowledge of available community services and supports.

II) Essential Functions

A) Provide ongoing support to individuals with developmental disabilities, aiding in the development of their Person Directed Plan (PDP) and initial Individual Plan of Care (IPC). This includes coordinating with internal departments, providers, and timely documentation.

B) Participate in a discovery process to identify personal goals, barriers, and necessary services.

C) Coordinate and facilitate the PDP process according to established timelines.

D) Develop narrative results of the PDP for everyone served, ensuring alignment with personal goals.

E) Address identified barriers to goal achievement and communicate with the individual and service providers accordingly.

F) Assist in the development of the IPC based on PDP results and ensure accurate documentation in the database.

G) Identify community resources or natural support before recommending HCS services.

H) Monitor individual progress toward desired outcomes and schedule planning activities as needed.

I) Conduct monthly monitoring of services provided to evaluate effectiveness.

J) Conduct quarterly reviews of progress towards PDP outcomes and facilitate revisions as needed.

K) Coordinate updates to the PDP based on the individual's changing wants and needs.

L) Ensure individuals are monitored according to established frequency guidelines.

M) Maintain accurate documentation, including current PDPs/IPCs and all required Medicaid documentation.

N) Ensure timely completion of administrative claiming and targeted case management documentation.

O) Review and indicate agreement/disagreement with recommended Level of Need assessments.

P) Build effective relationships with stakeholders, providing providers with current PDPs and addressing concerns discovered during monitoring.

Q) Assist in securing generic resources as needed and report pertinent information to program management.

R) Ensure timely communication of necessary information to providers.

IV) Internal & External Customer Service

A) This position requires extensive internal and external contacts. The employee will accomplish this with advanced written and verbal skills.

B) Must demonstrate compassion, trauma-informed care, and safety practices including suicide safe care.

V) Travel

A) Adhere to MHMR Mileage and Travel reimbursement policy and any other aspect regarding travel.

B) This position requires a minimum of 20% local travel to meet the needs of the community and may on occasion travel out-of-area for training and meetings.

B) Defined Education: Health and Human Services Acceptable Degrees Include: Applied Behavior Analysis, Behavioral Services field, Communication Disorders, Criminal Justice, Counseling, Education, Family Studies, Gerontology, Educational Psychology, Health Services, Human Services field, Human Growth and Development, Medicine, Nursing, Physician Assistant, Psychology, Rehabilitation, Social Services field, Social Work, Sociology, and Special Education

VIII) Agency Requirements

A) All staff are required to participate in agency Emergency Preparedness and Environmental Safety programs and may be assigned by their department as a key/essential staff level function during critical events or for the purpose of sustaining business continuity.

B) This position may require temporary or permanent re-assignment to any MHMR Tarrant facility as determined by program needs and/or the Division/Director.

C) Assigned work hours may change as the needs of the agency change.

D) The Functional Title of this position may change as the needs of the agency change.

E) All work will be completed within the scheduled work hours. All non-exempt (hourly) employees are expected to clock in and clock out for each work shift, no work should be done off the clock.

F) MHMR reserves the right to change, add to or eliminate positions as it deems appropriate.

G) Employment is at will, as well as agency needs may change.

H) Agency dress code is to be followed at all times.

I) Physical on-site presence, including regular attendance and punctuality, is an essential function of this position. Any changes or adjustments to your assigned work schedule or shift hours must be approved by your supervisor in advance.
