Registered Nurse

CenterPoint Health is seeking a compassionate and clinically skilled Registered Nurse to join our Healthcare & Medical Services team. As a key member of an integrated care organization providing primary care, dental care, and behavioral health services, you will play a vital role in supporting underserved communities by delivering accessible, affordable, and high-quality healthcare. In this role, you will provide direct patient care across the lifespan, including comprehensive assessments, development and implementation of individualized care plans, administration of medications and treatments, wound care, IV therapy as appropriate, and ongoing monitoring of patient status. You will collaborate closely with physicians, advanced practice providers, behavioral health specialists, dental providers, medical assistants, and care coordinators to ensure that patient needs are addressed holistically across medical, dental, and behavioral health domains. Your responsibilities will include obtaining histories and vital signs, performing focused and comprehensive nursing assessments, supporting chronic disease management for conditions such as diabetes, hypertension, and asthma, and providing patient and family education to promote self-management and preventive care. You will utilize Electronic Health Records (EHR) to document care accurately and timely, track test results, reconcile medications, and support care coordination, including referrals and follow-up outreach. Because CenterPoint Health is deeply committed to community health improvement, you will be expected to deliver culturally competent, patient-centered care, advocate for patients experiencing barriers to care, and participate in quality improvement initiatives that measure and enhance patient outcomes. You may assist with population health efforts such as outreach to high-risk patients, care gap closure, immunization campaigns, and health education initiatives. The Registered Nurse will also contribute to clinic flow and safety by following infection control and regulatory standards, participating in huddles and team meetings, and assisting with triage—both in person and via telephone—to determine the appropriate level and urgency of care. You will provide support for behavioral health integration by recognizing and escalating behavioral health needs, coordinating with behavioral health providers, and helping patients navigate available resources.
