Key Responsibilities

Provide case management services within an acute care hospital setting, managing a caseload of approximately 25 patients per RN
Collaborate with a multidisciplinary team including social workers and clinical staff to coordinate patient care and discharge planning
Utilize documentation systems including Meditech, Midas, and Wellsky to maintain accurate and timely patient records
Function independently while contributing to a team-oriented unit environment
Manage multiple priorities simultaneously while maintaining strong communication with patients, families, and care team members

Required Qualifications

Active Registered Nurse (RN) license valid in the state of Florida
Minimum of 2 years of experience in an acute care setting
Experience in hospital-based case management
Proficiency or ability to quickly learn electronic documentation systems (Meditech, Midas, Wellsky)
Strong time management, communication, and organizational skills

Preferred Experience / Skills

Previous travel nursing experience
Ability to adapt quickly to a fast-paced environment and work independently with minimal supervision
Positive, team-focused attitude with a reliable and flexible work style
