Job Description:

We are seeking a compassionate and dedicated Memory Care/Resident Care Coordinator for our assisted living facility. The ideal candidate will have a passion for working with individuals in need of memory care and ensuring they receive the highest quality of life possible in a supportive environment with activities throughout the day.

Responsibilities:

- Coordinate and deliver comprehensive care plans tailored to meet the individual needs of residents in the memory care unit.
- Collaborate with healthcare professionals, caregivers, and families to ensure optimal memory care support.
- Maintain and oversee the implementation of medication management and daily living assistance for residents.
- Develop engaging and therapeutic programs to enhance the cognitive and emotional well-being of residents.
- Conduct routine assessments and observations to monitor residents' progress and adapt care plans as necessary.
- Provide training and mentorship to caregiving staff to ensure the highest standard of care is delivered.
- Foster a warm, safe, and nurturing environment that respects the dignity and preferences of each resident.
- Act as a liaison between families and the facility, offering updates on residents' health and contributing to family meetings.
- Ensure compliance with all state and federal regulations pertaining to assisted living and memory care services.

**Qualifications:**

- Experience in a similar role within assisted living or memory care settings.
- Knowledge of dementia and Alzheimer’s disease and the best practices for their management.
- Excellent communication, leadership, and organizational skills.
- Ability to work collaboratively with a diverse team of caregivers and healthcare professionals.
- Certification in gerontology, nursing, or a related field is preferred.
- A compassionate and patient-centered approach with a commitment to providing exceptional care to residents.
