Hybrid Case Manager - Behavioral Health & Justice
Treatment Alternatives for Stronger Communities
TASC Case Manager will provide comprehensive case management services for adults and youths with Behavioral Health needs referred to TASC from multiple systems. The position requires travel to Will, Lake, McHenry, DeKalb, DuPage, Kankakee, Kendall and Kane counties and will be based in a hybrid work setting with some in-office and remote work. Essential duties include intake, referrals, crisis intervention, documentation, and collaboration with partners to support individualized service plans #J-18808-Ljbffr Treatment Alternatives for Stronger Communities
