The Blount Re-Entry Care Coordinator
Summary of role of team :
- The Blount County Transition Campus RRHP is classified as an "adult supportive residential facility" providing 24-hour residential care incorporating treatment and rehabilitation components. Services are provided to include the following: psychiatric services, nursing services, case management services, wrap-around and recovery services, transportation services and personal care services.
- The Program promotes training in community living skills and increased socialization opportunities through the provision of daily leisure and psycho-educational activities.
- The program is staffed with a minimum of one staff member for every twelve service recipients 24 hours per day/seven days per week/365 days per year.
- The program provides services to adults who are age 18 and above. Service recipients must be court ordered to the program.
Summary of position :
- Serves as the primary care coordinator that provides care coordination using an integrative approach to all clients in the RRHP Program.
- Functions as a member of a treatment team to plan, implement, and evaluate successful interventions for adults.
- Embraces the key values of empowerment, normalization, rehabilitation, and continuity of care.
- Provides advocacy, linkage, and referral services as needed.
- Interfaces with community agencies and referral sources to coordinate care.
- Provides therapeutic support and crisis intervention as needed.
- Mandatory to remain awake and alert during shift to monitor client safety.
- Upholds center policy and procedures, and CARF standards.
TYPICAL WORKING CONDITIONS/ENVIRONMENT
- Position is based within the RRHP Program, which is an Adult Supportive Residential facility that provides services 24/7.
- This positon requires frequent community involvement and group community outings.
JOB DUTIES/RESPONSIBILITIES

1. Quality Care
- Provides care coordination for clients with co-occurring mental health and substance use diagnoses
- Provides weekly face to face care coordination contacts for all clients in the RRHP Program.
- Facilitates care coordination between the primary care provider, behavioral health provider, pharmacy, and specialists regarding health needs of the client.
- Promotes health education with the client and/or family.
- Provides crisis intervention and emergency services as needed.
- Ensures and client participation in treatment planning as evidence by signatures on treatment plans.
- Attends and is timely to weekly multidisciplinary treatment team meetings
- Attends and participates in administrative/ staff team meetings without tardiness
- Responds to all flags, e-mails, and voicemails within 2 business days.
- Assists with the completion of the treatment plan within 48 hours of admission with input from other professionals
- Facilitates groups and organizes activities for groups on a daily basis.
- Performs urine drug screens as required.
- Collaborate with other staff to maintain participant's schedule and provide daily structure.
- Provide transportation and case management services in Blount County and possible surrounding counties to include employment placement, resources, and relapse prevention linkage to all required external services.
- Respects dignity and worth of each person served using trauma informed practices
2. Documentation requirements
- Completes all required clinical documentation and assessments in accordance with Center Policy and Procedure and funding source guidelines.
- Completes and signs EMR progress notes within two business days of encounter
- Completes group logs and other required documentation on time according to agency standards
- Ensures that all clients have an up-to-date ROI, care plan, crisis plan, consent to contact after discharge, DLA-20, Health Link Consent Form, and updated signature admissions page every 6 months, and as needed.
- Creates and coordinates treatment interventions that are reflected in progress note documentation based on the care plan goals
3. Team Atmosphere

- Interact and coordinate with law enforcement and other criminal justice agencies, as well as outside facilitators.

COMPENSATION:
Starting salary for this position is approximately $18.97/hour based on relevant experience and education.
Schedule:
The schedule of this position is 40 hours per week. Due to the diversity of job responsibilities, this position requires flexibilities in scheduling (days, evenings, weekends, and holidays).
Travel :
This position requires utilizing a personal dependable vehicle to conduct Center business and transport clients. Maintaining a dependable vehicle and certified driver status is a condition of employment. F Endorsement is required for this position and must be able to operate company owned vehicle as well.
Equipment/Technology:
Computer skills and ability to utilize company program software to properly document. Telephone and faxing skills. Must be efficient in computer skills. Use of Microsoft Office is required. Must be able to use a fax machine and phone system.
QUALIFICATIONS - Blount County Re-Entry Care Coordinator
Experience / Knowledge:
Must have course work and or experience in the areas of cultural diversity, human development, etiology and treatment of medical disorders and mental illness, alcohol and drug abuse, physical and sexual abuse, suicide, and intellectual disability. Computer experience is helpful.
Education / License :
Bachelor's degree in a health-related field of counseling, psychology or social work. Candidates with a Bachelor's degree in rehabilitation, occupational therapy, criminal justice, and education must have fifteen (15) college-level semester hours of course work in behavioral health and at least one (1) year of work experience in the behavioral health setting. This position requires utilizing a personal dependable vehicle to conduct Center business. Maintaining a dependable vehicle and certified driver status is a condition of employment.

- Ability to effectively and ethically coordinate care for children, adults, and/or families.
- Ability to present professionally and work within a team format to plan, implement, and evaluate successful interventions.
- Ability to work within a team format to meet positive goals for adults and/or children and interface with other agencies involved in the ecology of the individual.
- This position requires certification in and adequate implementation of verbal and physical de-escalation techniques that include a wide range of bodily movements including but not limited to grasping, holding another person, going down on knees, running, and walking.
- Must have mental ability to exercise sound judgment under pressure.
- Ability to exercise sound judgment and effective decision-making, ability to set and demonstrate appropriate boundaries, ability to be an empathic listener, flexibility, willingness, and adaptability to working with diverse populations.
- Must also have the ability to communicate effectively and possess good time management and organizational skills.
- Hearing of normal and soft tones.
- Close eye work.
- Valid driver's license- "F" endorsement.
- Lifting up to 50lbs.
