Hospice RN Case Manager - Full-Time

Role Overview Seeking a dedicated Registered Nurse Case Manager passionate about End of Life Care to support patients and families in a hospice setting. The role involves assessing patient needs and preferences, including emotional, spiritual, and cultural factors developing, implementing, and documenting personalized care plans in collaboration with an interprofessional team promoting respectful, equitable, and empathetic interactions with diverse populations evaluating patient condition changes, updating plans, and communicating with families and team members ensuring evidence-based practice and quality outcomes Key Responsibilities Assess patient preferences and barriers to involvement in care Develop and communicate individualized care plans Collaborate with families and team members to deliver holistic care Monitor and evaluate patient condition, adjusting care as needed Practice safety-sensitive procedures, maintaining alertness and safety at all times Qualifications & Skills Minimum of a Nursing Diploma/Associate's degree in Nursing At least 2 years of nursing experience; prior supervisor experience not required Current RN license and valid Driver's License Preferred: Bachelor's degree in Nursing or Home Health; 2-5 years of experience Additional Information The role includes coverage across all regions, with opportunities for career progression through a formal RN ladder, professional development, and competitive benefits.
