Remote Nurse Case Manager I | Care Coordination
Elevance Health
Elevance Health is seeking a Nurse Case Manager I for a fully remote, full-time role. The position supports care management for members with complex and chronic needs, via phone or on-site discharge planning, with mandatory in-person training sessions as required.
Minimum requirements include BA/BS in a health-related field, 3+ years of clinical experience, and an unrestricted RN license; multi-state licensure is required for multi-state service. Certifications and EST/CST preferences are noted.
