Responsibilities Assess the psychosocial status of patients and families/caregivers related to the patient’s terminal illness and environment, and communicate findings to the registered nurse and other interdisciplinary team members. Provide assessments in patients’ residences and assist when the setting is unsafe or another plan is required. Carry out social evaluations, including family dynamics, caregiver abilities, communication patterns, and assess risk for suicide, neglect or abuse, then plan interventions based on findings. Provide counseling to patients and families/caregivers as needed for stress and coping difficulties; offer crisis intervention when necessary. Assess and educate the interdisciplinary group on any special cultural needs of patients and families, including communication styles, family roles, space, and traditions or taboos. Maintain clinical records for all patients referred to social work. Educate patients and families on advanced directives and assist in preparation. Provide information and referral services for patients and families regarding practical and environmental needs. Serve as liaison between patients/families and community agencies. Maintain collaborative relationships with internal personnel to support patient care. Maintain and develop contacts with public and private agencies as resources for patients and personnel. Participate in developing individualized care plans, attending interdisciplinary meetings, and assisting the team in recognizing psychosocial stress effects on symptoms of terminal illness. Assist physicians and team members in understanding significant social and emotional factors related to health issues and death/dying. Actively participate in quality assessment, performance improvement teams, and activities. Assist families and patients in planning for funeral arrangements, financial, legal, and health‑care decision responsibilities. Perform other duties as delegated by the Clinical Supervisor. Requirements Master’s degree in Social Work from an accredited university (students with a B.A. in Social Work are eligible; pay will be based on degree type). Minimum of two (2) years of documented supervised experience in health care; hospice experience preferred. Understanding of hospice philosophy and issues of death/dying. Certified Hospice and Palliative Social Worker designation is desirable. Flexible in work hours and ability to travel within the assigned Bristol Hospice service area. Willingness to maintain comprehensive working knowledge of information systems and applicable software programs.
