Physical Therapist

Role Overview: This position involves providing personalized, one-on-one physical therapy to older adults in their homes or senior living communities, supporting mobility, independence, and quality of life through evidence-based treatments and evaluations. Responsibilities Include Delivering therapy sessions in various settings, including private residences and senior facilities Developing individualized care plans and documenting in electronic medical records Collaborating with an interdisciplinary team of professionals Educating patients, caregivers, and staff on safety and aging in place Utilizing telehealth and virtual care to extend access and flexibility Qualifications And Skills Valid Physical Therapy license or eligibility in applicable state(s) Degree from an accredited program Proficiency with computers and electronic medical records Current CPR certification Physical Requirements: Ability to perform moderate physical effort, assist with patient transfers, stand and walk for extended periods, climb stairs, and drive to patient locations. PPE use is required when necessary.
