Preferred two years of experience in acute or sub-acute care setting. Must meet credentialing requirements for Medicare, Medicaid, and Managed Care Organizations. Evaluation and management of clients with multiple medically complex health conditions. Ability to function autonomously in a fast-paced environment. Superior Communication skills. Excellent organizational skills and effective time management capabilities. Collaborates with facility personnel to ensure multi-disciplinary approach. Desire to provide ongoing education to LPNs/RNs on a routine basis.Maintains positive and professional attitude and demeanor.
