Description

• Conduct thorough evaluations of patients in both inpatient and outpatient settings and formulate personalized treatment plans
• Provide physical therapy interventions geared towards mobility, strength enhancement, balance restoration, and functional improvement
• Collaborate with nurses, physicians, case managers, and various rehabilitation professionals to maximize patient outcomes
• Educate patients and their families on therapeutic objectives, discharge plans, and at-home exercise routines
• Maintain accurate documentation of evaluations, treatments, and progress per facility and regulatory guidelines
• Engage in care team rounds and interdisciplinary meetings to ensure cohesive care

Education

• Doctorate in Physical Therapy (DPT) or an equivalent qualification from an accredited institution

Certifications

• Valid state licensure as a Physical Therapist
• CPR certification is preferred
• Additional certifications in specialties such as neurological or geriatric care are beneficial but not mandatory
