This position involves evaluating patients, developing and implementing treatment plans, and ensuring safe and effective discharge planning. The role emphasizes professional growth, patient safety, and adherence to organizational standards. Job Details Schedule: Days shift, 8 hours per day, with a minimum of 40 hours per week. Certifications, Education Requirements, Degree Requirements, Location Requirements, and Years of Experience: Degree in Physical Therapy from an Accredited School; 1 Year of Outpatient Clinical Experience; CPR Certification; active applicable license for the state where work will be performed. Responsible for performing comprehensive patient evaluations and developing individualized treatment plans aimed at progressing patients toward discharge within an established length of stay. Completes medical record documentation accurately, including goal setting, functional assessments, letters of medical necessity, and charge entry, in compliance with regulatory guidelines and organizational policies. Provides supervision and mentorship to students, fostering a positive learning environment. Actively pursues ongoing professional development through continuing education and engagement with professional organizations. Ensures all care is delivered under signed, valid physician orders and within insurance authorization parameters. Adheres to the organizations performance improvement initiatives, contributing feedback to maximize quality outcomes. Demonstrates adherence to all safety policies, including safe patient handling and infectious disease protocols. Maintains professional licensure in good standing and completes all state-mandated continuing education requirements. Demonstrates knowledge of the organizations performance improvement methodology and applies it to daily practice.
