Wellness Specialist I (MA) Full‑time Salaried position located in Harlem, 2090 Adam Clayton Powell JR Blvd, New York, NY 10027. Weekly hours: 35, Monday to Friday 9:00 AM to 5:00 PM (including one remote day per week). Salary: $68,959.00 to $72,000.00 annually (based on qualification: LMSW/MHC-LP to LCSW/LMHC). Shift Hours: 9:00 AM – 5:00 PM, Monday‑Friday with one remote day per week. Location: 2090 Adam Clayton Powell Jr Blvd, New York, NY 10027. Workplace Flexibility: Flexible; staff may start remotely one day per week. Job Summary CASES seeks a Master’s-level mental health clinician to join the Forensic Assertive Community Treatment (FACT) team, providing outreach, engagement, assessment, and psychoeducation in the community. The role supports individuals aged 18+, with severe mental illness, substance use disorders, and criminal legal system involvement, using an evidence-based ACT model to promote recovery and stability. Responsibilities Provide 80% of treatment contacts in the community and meet minimum monthly contact requirements, including leading clinical groups when indicated. Attend and participate in daily team meetings to discuss client clinical and case management issues. Complete required documentation per NYS Office of Mental Health (OMH), NYC Department of Health and Mental Hygiene (DOHMH), NYS Medicaid billing guidelines, and CASES ACT Policy & Procedure. Collaborate with clients’ support networks and other service providers on treatment planning, service coordination, and case management. Offer clinical services that are anti‑racist, gender‑responsive, culturally humble, trauma‑informed, recovery‑oriented, client‑centered, and evidence‑based. Complete documentation and toxicology tests for clients with active Assisted Outpatient Treatment (AOT) orders. Provide 24‑hour crisis intervention on‑call services on a rotating basis. Deliver culturally competent services in line with CASES policies and practice. Qualifications Current New York State license: LMHC, MHC‑LP, LMSW, or LCSW. Graduate‑level education (Master’s degree in social work or related field). Proficiency in computer use. Strong organizational and self‑motivated work style. Experience with evidence‑based treatment models for substance use and mental illness. Individuals with lived experience are encouraged to apply. Preferred Skills Fluency in Spanish.
