• Deliver individualized and group therapeutic interventions that address the specific needs of clients, drawing from evidence-based practices.

• Conduct comprehensive psychosocial assessments and mental health intake evaluations to ensure personalized care planning.

• Provide essential social work services, supporting clients and their families through challenges associated with mental health and chronic conditions.

• Collaborate with a multidisciplinary team of healthcare professionals to coordinate holistic patient care, ensuring seamless communication and support.

• Develop, implement, and continuously monitor patient treatment plans, documenting progress and making necessary adjustments as needed.

• Prepare and maintain accurate clinical documentation including treatment records, session notes, and progress reports, adhering to legal and ethical standards.

• Engage in ongoing team meetings and training sessions to enhance service delivery and contribute to a culture of continuous improvement.

• Fulfill additional responsibilities as assigned by management that align with organizational goals and values.

Your primary focus will be on providing care to a diverse patient population experiencing chronic conditions that impair their mental health. By leveraging your expertise, you will help clients navigate their personal challenges and empower them towards improved emotional and psychological resilience.

• Master’s degree in Professional Counseling from an accredited institution.
\n
• Current and active license as a Licensed Professional Clinical Counselor (LPCC) in the state of California.

• A minimum of 3 years of direct experience providing counseling services in a clinical setting.

• Exceptional responsiveness and operational skills, demonstrating the ability to manage caseloads effectively.

• Proficiency in utilizing technology for telehealth services, with the ability to provide constructive feedback to improve workflows.

• Strong crisis management skills, equipped to handle emergencies and provide adequate support.

While the minimum experience requirement is three years, those with additional experience in specialized areas such as trauma-informed care, cognitive behavioral therapy (CBT), or family therapy will be highly regarded. Familiarity with telehealth platforms and innovative therapy models is a plus.

The annual salary for this position ranges from $120,000 to $130,000, supplemented by a robust benefits package.
