Compensation

• $34 – $47/hour (based on experience)
• $10,000 Sign-On Bonus
• Relocation Assistance Available (case by case)

Position Overview

We are seeking an experienced RN Case Manager to join a leading acute care hospital in the Florida Panhandle. This role is ideal for a clinically strong nurse who is passionate about patient advocacy, discharge planning, and coordinating safe, high-quality care throughout the patient journey.

The ideal candidate is organized, confident communicating with physicians and families, and thrives in a collaborative hospital environment.

Key Responsibilities

• Coordinate patient care plans across the healthcare continuum
• Manage discharge planning and psychosocial assessments
• Educate patients and families regarding treatment goals and post-discharge care
• Collaborate with physicians, nurses, and interdisciplinary teams
• Advocate for safe and appropriate patient outcomes
• Ensure timely documentation and care coordination
• Support patients and caregivers through rehabilitation and recovery planning

Qualifications

• Active Registered Nurse (RN) license
• Associate Degree or Nursing Diploma required
• Minimum 2 years of recent acute care experience within the last 3 years
• Background in ICU, CCU, Med-Surg, LTAC, or Hospital Case Management preferred
• Strong communication, critical thinking, and organizational skills
