Case Manager - Registered Nurse (RN)
We are seeking an experienced Registered Nurse (RN) to join our team as a Case Manager. This role involves managing patient care efficiently and ensuring quality outcomes in an acute care environment.
Key Responsibilities
- Coordinate and manage patient care plans from admission to discharge.
- Conduct assessments and develop individualized care plans by collaborating with healthcare teams.
- Monitor patient progress and adjust care plans as needed.
- Utilize knowledge of Meditech, Midas, and Interqual in managing cases effectively.
- Serve as a resource for patients and their families regarding healthcare options and resources.
- Ensure compliance with regulatory standards and quality benchmarks.
Qualifications
- Must possess a valid Registered Nurse (RN) license.
- Minimum of 2 years' experience in acute (hospital) case management required.
- Demonstrated knowledge and proficiency in Meditech, Midas, and Interqual systems.
- Strong communication and interpersonal skills, with a focus on collaboration.
Work Schedule
This position requires availability for every other weekend.

Position is based in Fort Walton Beach, FL.
