Clinical Social Worker

Job Description The incumbent will be assigned to Citywide Case Management Forensics Program. In this position, the incumbent will collaborate with criminal justice partners including Jail Behavior Health, NoVA, Behavioral Health Court and Mental Health Diversion Court as well as SFGH Psychiatric Emergency Services in working with dually diagnosed adults in the community. The incumbent will provide comprehensive assessment, crisis intervention, case management, individual and group therapy, and substance abuse services. Clients will receive their mental health and substance abuse services at the Center or wherever they are located. The incumbent will assist clients in meeting their needs and will make referrals as appropriate. Our programs are part of the SFGH Department of Psychiatry's comprehensive effort to treat high-risk clients in the community and to reduce unnecessary use of institutional care. Service Provision Provides ongoing mental health services, including psychotherapy (Individual, Group, & Family), Crisis Intervention, Case Management and Outreach to a caseload of severely mentally ill clients. Makes case terminations and discharge plans, when clinically appropriate. 15%
Service Planning, Consultation and Liaison Develops a comprehensive individual service plan for each client, integrating knowledge of a wide range of human service resources. Provides consultation to a multi-disciplinary treatment team on the psychosocial ramifications of psychiatric illness. Facilitates treatment coordination among various agencies and advocates for and links clients to needed services. Provides consultation to and maintains liaison with campus and community programs, including educational, legal, and social organizations involved in case management. Acts as liaison to inpatient units to facilitate referrals and service treatment. 10%
Intake/Assessment Screens referrals for meeting program criteria; conducts intake of new clients, including assessment, community functioning, and service needs, when appropriate. This entails assessment of presenting problems, psychosocial history, mental status and diagnosis (DSM-IV). Responds to unscheduled "walk in" clients, utilizing crisis intervention techniques and providing 5150 services as needed. Serves as weekly OD screening clients for entry into the clinic. 10%
Record Keeping and Administration Maintains client's records according to California, City and County requirements. Records data in client's charts. Meets program productivity standards. Assists in processing quality improvement requests. Completes billing and other documentation of services in a timely fashion. Attends staff and clinical team meetings, and participates in training sessions (e.g., In services, grand rounds). Identifies program problems and makes recommendations to supervisory and administrative staff. 5%
Education and Training Provides education to the community regarding program services, client needs and treatment. Assists in planning and providing in-service to staff. Provides orientation and didactic training to interns. If desired and appropriate, supervises student(s) and participates in student training program meetings. 100%

- CSW I: Master's degree in social work, psychology or related field from an accredited university and registered with the Board of Behavioral Science as An Associate Clinical Social Worker (ASW) or Intern Marriage Family Therapist (IMFT). Plus all of the above.
- CSWII: Master's degree in social work, psychology or related field from an accredited university and possession of the requirements necessary for application for licensure as determined by the California Board of Behavioral Science. Plus all of the above.
- Registration with the CA Board of Behavioral Sciences
- Two years working with severely mentally ill adults, the homeless and dual diagnosed populations
- One year experience doing community-based case management and the ability to do outreach including to SRO hotels
- One year experience working with individual actively involved in the criminal justice system
- Ability to obtain San Francisco County Jail Clearance through the San Francisco Sheriff's Department
- Ability to work cooperatively and communicate effectively with others. Ability to work in a high stress, high volume area and work well under pressure and with deadlines. Ability to follow oral and written instructions accurately, and be detailed oriented. Able to efficiently prepare written summaries of activities, set priorities and complete reports within required timeframes. Knowledge of medical and/or psychiatric terminology

- Demonstrated excellent attendance and reliability
- A preliminary job offer and assignment may be extended pending successful background clearance.

- Prior related UC experience
