Job Summary:
The Registered Nurse - Case Manager provides skilled nursing care with a focus on care coordination and discharge planning within an acute care hospital setting. This role requires at least two years of acute care hospital experience and involves collaborating with interdisciplinary teams to ensure effective patient management. Weekend rotation and adherence to state licensure requirements are essential.

Responsibilities:
- Perform care coordination and discharge planning for patients in an acute care hospital.
- Conduct patient assessments and administer medications as prescribed.
- Develop and implement treatment plans in collaboration with interdisciplinary teams.
- Communicate effectively with patients, healthcare providers, and team members to ensure quality care.
- Maintain accurate and timely documentation of patient care and case management activities.
- Adhere to hospital policies and procedures, including infection control and safety standards.
- Participate in weekend rotation and meet departmental staffing needs.
- Active California Registered Nurse license (pending license accepted if licensed upon start date).
- Minimum of 2 years acute care hospital experience in case management, care coordination, and discharge planning.
- Strong clinical assessment, medication administration, and treatment planning skills.
- Excellent communication and interpersonal skills.
- Ability to work effectively in a fast-paced acute care environment.
- Bachelor's degree in Nursing or related field preferred.
- Experience with hospital case management systems and electronic medical records.
- Previous experience working as a travel nurse or in multiple hospital settings.
- Business attire or scrub wear flexibility.
- Weekend rotation required to meet departmental needs.
- No floating expectation.
- Patient-to-nurse ratio approximately 1:20.
- Return to office (RTO) restrictions allow no more than 7 days.
- Work environment is an acute care hospital.
- Compliance with systemwide Do Not Return (DNR) policy if cancellations occur post-signature.
- Work schedule includes weekend rotations.
- Shifts assigned based on departmental requirements.
- Start date as soon as possible.
