Visiting Nurse & Hospice of Litchfield County is in search of a Licensed Clinical Social Worker (LCSW) to join our team covering the Torrington & Greater Farmington Valley area.
We provide Hospice and Home Care services to the residents of Litchfield County and Farmington Valley with a talented and experienced clinical team. The social worker as a member of the care team assists in long-term planning, provides education and assists patient and family members to access community resources to facilitate home safety. The social worker is a key member of our hospice and palliative care teams and must be comfortable with end-of-life care.
EDUCATION AND/OR EXPERIENCE REQUIRED:
· Master’s degree in social work from accredited graduate program
· Experience in home care and hospice preferred
LICENSE AND/OR CERTIFICATIONS REQUIRED:
· Licensure by CT State Department of Public Health as a Clinical Social Worker
