WeCare at Foltsbrook is actively seeking motivated Licensed Practical Nurses for our community in Herkimer, NY. Our Licensed Practical Nurses assist in ensuring the general health and well-being of our residents by providing nursing care. The Licensed Practical Nurse (LPN) provides nursing care under the supervision of a registered nurse (RN) or physician to residents. The LPN assists in implementing the nursing care plan, administering medications, and providing basic medical care to residents.

Key Responsibilities

·      Conduct thorough assessments of each patient’s medical status upon admissions and throughout the course of treatment.

·      Provide regular patient status updates.

·      Assist in implementing patient’s treatment plan under direction of the Registered Nurse.

·      Complete required documentation in an accurate and timely manner.

·      Supervise, administer and or assist with medications.

·      Provide direct care of residents as necessary.

·      Follow the plan of care for each resident.

·      Ensure the residents care standards are maintained.

·      Report to the Director of Nursing.

Skills, Knowledge and Expertise

·      Valid NYSED LPN Certification

·      Basic Life Support – American Heart Association (BLS)
