Position Snapshot
Location: Must reside in Macomb or the surrounding areas in Illinois (Must own or rent your current home; Only available in the following counties: Fulton, Hancock, Henderson, Knox, McDonough, Schuyler, and Warren)
Supporting: You are welcoming an individual with an intellectual disability to move into your home and live with you full-time as a part of your household and family dynamic
Average Annual Income: Approximately $33,000 (federally tax-free) + Monthly Rent & Household Expenses Contribution

How Youll Make a Difference
Every individual is unique, but your responsibilities may include:
Supporting daily living skills such as cooking, budgeting, cleaning, and decision-making.
Assisting with medication administration and medical appointments.
Helping someone build connections, participate in their community, and experience a sense of belonging.
Providing transportation and support with daily activities as needed.
Maintaining documentation and communication with the Mosaic support team.
Helping someone build confidence, develop independence, and pursue personal goals.
Depending on the individuals needs, support may also include:
Assistance with personal care activities
Behavioral support
Medical or physical support needs
Specialized care and monitoring

Requirements
Must be at least 21 years of age
You must be a current resident of the state of Illinois (due to state funding restrictions).
Own/Rent your own home, apartment, condo, or townhouse
Active homeowners or renters insurance
Reliable transportation, valid drivers license, and current vehicle insurance
Internet access and basic computer skills
Ability to complete required screenings, background checks, and training
Must be a registered healthcare worker (DSP)
