Compassionate Hospice RN Case Manager
Gentiva
Gentiva Hospice is seeking a dedicated RN Case Manager to lead comprehensive in-home hospice care, assess patients, and coordinate interdisciplinary teams to deliver compassionate, personalized support for patients and families at home or in other settings.
The role requires an RN license, 1+ year of RN experience in hospice/home health, CPR, and a strong focus on education, comfort, and quality care. This is a safety-sensitive, full-time opportunity with comprehensive benefits.
