Senior Primary Care NP | Value-Based Practice

Oak Street Health is seeking a Nurse Practitioner to deliver effective primary care to adults on Medicare in our outpatient clinics. You will practice independently within a collaborative care team including physicians, NPs/PAs, medical assistants, a scribe, RNs and care managers. You will lead wellness visits, health risk assessments, and care coordination to improve population health and patient outcomes while building lasting patient relationships and ensuring high-quality documentation. #J-18808-Ljbffr Colorado PA
