Registered Nurse

Seeking an experienced RN Case Manager to support acute care case management, focusing on patient care coordination, discharge planning, and regulatory compliance in a hospital setting. Key Responsibilities Conduct admission and ongoing case management assessments (medical necessity, level of care). Drive discharge planning and reduce length of stay through care coordination. Ensure CMS compliance, including admission reviews, continued stay, IMM & MOO. Coordinate transitions of care and post-acute placement. Maintain accurate and timely clinical documentation. Collaborate with physicians, nursing, and interdisciplinary team. Identify high-risk patients and prevent readmission. Support audits, compliance, and regulatory standards. Required Qualifications Active RN license. Case Management experience in an acute hospital setting. Experience with Cerner EHR. Key Skills Strong care coordination and discharge planning.
