Home Health Nurse Practitioner

We are seeking experienced Nurse Practitioners (NPs) and Physician Assistants (PAs) for flexible Home Health Assessment opportunities. Position Highlights Home Health Setting Flexible Scheduling Full-Time or Part-Time Available Daytime Hours Available 7 Days a Week Competitive Compensation Responsibilities Conduct pre-scheduled in-home health evaluations for Medicare Advantage, Medicaid, and select commercial health plan members. Visits include: Comprehensive medical and medication history review Physical health assessment Completion of patient-specific documentation provided prior to each visit This is a non-treatment role: No prescribing medications No ordering labs or diagnostic testing No modifying existing treatment plans Schedule & Volume Flexible scheduling options Average of 6 9 patient visits per day Requirements Active NP or PA license in the state of practice Board Certification Medicare/Medicaid eligible Credentialing Approximately 30-day credentialing process This opportunity is ideal for providers seeking supplemental income, schedule flexibility, or a rewarding role focused on preventative and wellness assessments.
