Responsibilities Provide high-quality preventative care to residents in skilled nursing facilities, utilizing remote patient monitoring, chronic care management, and collaborative care management approaches Conduct on-site visits and work closely with a dedicated scribe team for efficient documentation Build strong relationships with facilities and maintain a patient-centered approach Collaborate with other team members to ensure seamless care coordination Participate in quality improvement initiatives and contribute to enhancing patient outcomes Requirements Active NP or PA license in the state of practice 1+ year of experience in Geriatric care and documentation review Excellent communication, collaboration, and teamwork skills Strong problem-solving and critical thinking abilities Additional Benefits Hybrid role with flexible scheduling options Support and training from our clinical directors, regional managers, and supervisors Free CEUs Referral Bonuses Why Join Carease Health?
