Valor Hospice seeks a Patient Care Manager who is a registered nurse to ensure quality home-based patient care. The role oversees clinical departments, staffing and productivity to maintain high standards. The position coordinates care with the interdisciplinary team, patients, families and referring agencies, and monitors QAPI issues while guiding clinical staff on hospice policies.
