Physical Therapist Assistant

Description Physical Therapist Assistant (PTA) | Home Visits | Grand Rapids Area Atrio Home Care, a partnership between Holland Home, Brio, and Resthaven, is seeking a compassionate and driven Physical Therapist Assistant (PTA) to join our Grand Rapids team. In this role, you'll provide high-quality, one-on-one care to clients in their homes within approximately a 40-mile radius of Grand Rapids. What You'll Do
- Travel to client homes or care facilities to complete routine follow-up visits
- Deliver physical therapy services under the supervision of a licensed Physical Therapist, including implementing restorative care and home exercise programs
- Complete accurate clinical documentation and progress notes inMatrixCare, ensuring compliance with agency policies and HIPAA standards
- Collaborate with therapists and the interdisciplinary team to support excellent patient outcomes and smooth discharge planning
- Educate clients, families, and caregivers on therapy techniques and ongoing care needs
- Assist with therapy equipment management, program development, and participate in team trainings and goal-setting initiatives
- Hours: Full-time and part-time opportunities available
- Schedule: Monday-Friday (no weekends, holidays, or on-call requirements)
- Shift: 8:00 AM - 5 PM
- Coverage Area: Grand Rapids team
- Active Michigan PTA license required
- 1+ year of PTA experience preferred
- Experience working with geriatric or medically complex populations strongly preferred
- Strong ability to work independently while also collaborating as part of a team
- Excellent time management and organizational skills
