Job Description

Job Summary:
A Physical Therapist responsible for evaluating, treating, and rehabilitating patients with physical functioning disorders and wound care needs. The role involves developing individualized plans of care, utilizing evidence-based practices, and ensuring compliance with regulatory standards.

Responsibilities:

• Screen, evaluate, and implement treatment plans for patients with physical disabilities or wound care requirements.
• Administer procedures including manual therapy, therapeutic exercises, modalities, and use of assistive devices.
• Monitor patient progress, adjust treatments, and document outcomes accurately.
• Provide education, counseling, and communication to patients, families, and care teams regarding treatment plans and progress.
• Collaborate with healthcare team members and supervise Physical Therapy Assistants, aides, or students as needed.
• Maintain comprehensive documentation and participate in clinical meetings, trainings, and required conferences.

Qualifications:

• Hold an active State of Missouri Physical Therapist license.
• Possess at least 1 year of PT experience, including LTC or rehab.
• Be a graduate with Level II experience if a new graduate.
• Demonstrate proficiency in regulatory standards, documentation, and clinical assessments.
• Ability to obtain necessary DME and supplies per clinical requirements.
• Adhere to APTA and state practice acts, ethics, and professional conduct.
