Description

• Conduct thorough evaluations of patients in both inpatient and outpatient environments and craft personalized treatment plans
• Deliver engaging physical therapy interventions targeting mobility, strength, balance, and overall function
• Collaborate with fellow nurses, physicians, case managers, and rehabilitation experts to maximize patient outcomes
• Guide patients and their families in understanding therapeutic objectives, discharge strategies, and at-home exercises
• Document evaluations, treatment sessions, and progress meticulously, adhering to facility and regulatory standards
• Participate in team rounds and interdisciplinary discussions to ensure seamless and coordinated care

Education

• Doctorate in Physical Therapy (DPT) from an accredited institution or equivalent degree

Certifications

• Valid state licensure as a Physical Therapist
• CPR certification preferred
• Additional specialized qualifications, such as in neurology or geriatrics, are beneficial but not essential

Skills

• Prior experience in a physical therapy setting strongly preferred
