Housing Stability Case Manager

LANTERN COMMUNITY SERVICES, INC

Lantern Community Services, a Bronx-based nonprofit, seeks a dynamic Case Manager to support individuals with chronic medical conditions and housing stability. You will assess, plan, link to medical and social services, and document progress within an interdisciplinary team. The role emphasizes compassionate, proactive work with residents, including those affected by HIV/AIDS, mental illness, and substance use, to help them achieve physical, emotional, vocational, educational, and social goals. #J-18808-Ljbffr LANTERN COMMUNITY SERVICES, INC
