Nurse Practitioner

Main Responsibilities Conduct comprehensive in‑home evaluations and examinations. Use both in‑person and telemedicine capabilities to deliver care. Develop and implement personalized treatment plans. Manage acute clinical issues in the home. Maintain accurate documentation. Participate in medical team meetings. Report to the Chief Medical Officer for San Antonio, Texas. Work onsite by visiting patient’s homes in San Antonio, Texas. Undergo TB screening test if selected. Required Qualifications Registered Nurse Practitioner or Physician Assistant with 1+ year(s) of experience in Home Health or related field. Preferred Qualifications Bilingual in Spanish and English. Additional Information Language proficiency testing is required for any associate who speaks with a member in a language other than English. Applicants must take the Interagency Language Rating (ILR) test as provided by the Federal Government. Schedule and Pay Weekly hours: 40. Pay range: $123,800 - $170,300 per year (depending on geographic location and experience). Eligible for a bonus incentive plan based on performance.
