Job Title: Hematology/Oncology Advanced Practice Provider

- Proficiency in using Epic EMR for coordinated care
- Strong teamwork skills for collaboration with various healthcare professionals


Description:
This position is focused on outpatient oncology practice with inpatient collaboration as needed.
The advanced practice provider will manage patients with solid tumors and hematologic malignancies.
The provider will work closely with hematologists, oncologists, infusion teams, nurse navigators, and support services.
An established support staff will be available to assist in patient care.
On average, the provider will see 11-16 patients per day.
The patient population will include individuals of all ages.
This role involves both outpatient services and multidisciplinary care team collaboration.
Integrated support services include coordination of care, patient education, and behavioral health.
The provider will have access to oncology supportive therapies and clinical trials.
Opportunities for palliative care and genetic counseling will be offered.
The practice environment includes dedicated clinic staff, exam rooms, and an infusion center.
Effective management of chemotherapy, immunotherapy, and other oncology therapies is expected.
Oncology nurse navigation will support patient education and care coordination.
