Wider Circle seeks a Care Advocate to join its mission-driven team supporting WiderCare+ and CHI/PIN initiatives. You will collaborate with clinicians and the care delivery team to build trust, promote health, and coordinate care for high-risk patients across Texas, including remote work and local home visits within a 50-mile radius where required.
