Physical Therapist - Travel Contract

Minimum Requirements
Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the American Physical Therapy Association or equivalent accrediting body.
Current, unencumbered Oregon physical therapist license required prior to the start of the assignment.
Active BLS/CPR certification from the American Heart Association or American Red Cross required.
Home health experience preferred. 2–3 years of physical therapy experience preferred.
Experience with home evaluations, development and updating of plan of care, patient and family education, and documentation of patient progress required.
Experience supervising and evaluating PT assistants and home health aides preferred.
Must have reliable transportation and a valid driver’s license. Agency-required liability insurance as required by client policy.
Proficiency with basic computer functions, including email, electronic health record navigation, and online training platforms required.

Schedule: 5x8 shifts
Role Responsibilities: Perform patient evaluations, deliver therapeutic services per physician orders, document patient condition, advise and instruct family and agency personnel, evaluate home environment, participate in in-service education.
Supervisory Responsibilities: May directly supervise and evaluate Physical Therapy Assistants and Home Health Aides as assigned.
